Consent for Support Choices to Act on My Behalf

I give my permission for Support Choices to support and advocate for me where appropriate.

By signing this form, I authorise Support Choices to act on my behalf in relation to the issues for which I am seeking support. This may include:

• Attending meetings with me or on my behalf.
• Contacting professionals, organisations, or services involved in my situation.
• Speaking, writing, or communicating with relevant individuals and organisations.
• Requesting access to information, records, documents, or other materials that I receive, whether these are held in paper, electronic, or any other format.

Support Choices agrees to:

• Remain independent of service providers and avoid conflicts of interest.
• Support me to express my views and have my voice heard.
• Provide information and guidance to help me make informed decisions about my situation and possible options.
• Keep me updated about any significant developments or information relating to my care.

Support Choices cannot:

• Guarantee a particular outcome or result.
• Ensure that any decisions will be made in my favour.
• Take responsibility for commitments or agreements I make.
• Provide legal representation or take legal action on my behalf, although they may help me access legal advice or representation where appropriate.

Right to Withdraw Consent
I understand that I have the right to withdraw this consent at any time.
I acknowledge that Support Choices cannot continue to provide me with the services that I withdraw consent for.
I understand that this consent remains in force until the issue(s) has been/have been resolved or until I notify Support Choices that this consent is withdrawn, whichever happens first.

Privacy and Data Protection
The law says we must get your permission to store information about you (Data Protection Act 2018).
The way we collect, use, store, and share your information is governed by the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. We will treat information confidentially unless required by law to do otherwise. We may need to use personal information in order to act on the person's behalf with a relevant third party. Information will be kept confidentially by Support Choices while support is being provided. A more detailed data protection policy is available on request.
