I am the parent/legal guardian of the player named above and give permission for my child to participate in Shore FC’s open training sessions. I understand that soccer involves risks, including falls, collisions, and serious injury, and voluntarily accept these risks. My child is able to participate, and I will notify the coaching staff of any relevant medical conditions or restrictions.
To the fullest extent permitted by Maryland law, I release Shore FC, its coaches, staff, and volunteers from claims arising from my child’s participation, including claims caused by their ordinary negligence. This release does not apply to gross negligence, reckless conduct, or intentional misconduct.
If I cannot be reached in an emergency, I authorize staff to obtain emergency medical care for my child. I understand that I am responsible for associated medical expenses.
