Participant Liability Waiver, Assumption of Risk, Medical Authorization & Media Consent

Event: Thanksgiving Break Basketball Camp
Dates: November 23–25, 2026
Location: BTrained, 4900 Westside Rd, Redding, CA

1. Acknowledgment of Risk

I understand that participation in basketball and related athletic activities involves inherent risks, including but not limited to falls, collisions, sprains, fractures, concussions, and other injuries, including serious injury.

I voluntarily authorize my child(ren) to participate in the Next Level Athletics Thanksgiving Break Basketball Camp and acknowledge these risks.

2. Release of Liability

To the fullest extent permitted by California law, I, on behalf of myself and my participating minor child(ren), release Next Level Athletics, BTrained, and their respective owners, directors, coaches, employees, volunteers, and representatives from claims arising from the ordinary negligence of the released parties in connection with participation in camp activities or use of the facility.

This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that cannot lawfully be waived.

3. Medical Authorization

In the event of an injury or medical emergency, I authorize camp staff to contact emergency medical services and arrange appropriate medical care for my child if I cannot be reached promptly.

I understand that I am responsible for medical expenses incurred in connection with my child's treatment, to the extent permitted by law.

4. Health and Safety

I agree to disclose relevant medical conditions, allergies, injuries, or restrictions that may affect my child's safe participation. I understand that camp staff may restrict participation when reasonably necessary for safety.

5. Parent/Guardian Consent

I certify that I am the parent or legal guardian of the registered child(ren), that I have read and understand this agreement, and that I voluntarily consent to their participation.

I understand that this agreement may affect legal rights, including the right to bring certain claims.

6. Photography & Video Consent (Optional)

I understand that Next Level Athletics and BTrained may take photographs and record videos during the Thanksgiving Break Basketball Camp.

If I grant permission below, I authorize Next Level Athletics and BTrained to photograph, film, and use images or recordings featuring my child(ren) for promotional and informational purposes, including social media, websites, flyers, advertisements, and other marketing materials.

I understand that these materials may be shared publicly, that no compensation will be provided, and that I may withdraw permission for future use by contacting Next Level Athletics. I understand that previously published materials may not be fully removable.

7. Parent/Guardian Acknowledgment

By clicking below, I acknowledge that I have read and understand Sections 1–5 of this agreement, voluntarily authorize my child(ren) to participate, and agree to the terms to the extent permitted by applicable law.
