Acknowledgement of Risk & Medical Clearance
By signing below, I acknowledge that I have received a copy of, read and understand, the NSL Trial Policy and Agreement in its entirety.
I understand and accept that participating in Club activities, including but not limited to training sessions, scrimmages, and friendly matches, carries inherent risks, including injury, illness, and unforeseen accidents.
I confirm that I have received medical clearance from a licensed medical professional, affirming that I am physically fit and capable of participating in professional-level training.
Waiver of Liability
The undersigned Trial Player acknowledges and understands that dangers of personal injury are inherent in their participation in the sport of soccer whether in a match, scrimmage, training session, clinic or demonstration and the Trial Player expressly and voluntarily assumes all risk of personal injury or death sustained while participating in such activities, including the risk of hidden, latent or obvious defects in any of the facilities, or equipment used. Further, the Trial Player acknowledges the possibility that they or their successors may not fully know the number or magnitude of all the potential liabilities, claims, demands, cost, loss expense, or cause of action (together, "Claims"), but nevertheless intends to assume all risk of such injury or death. In connection with the foregoing, the Trial Player forever releases and discharges the Club, the NSL, its affiliated entities and other NSL clubs and their respective officers, directors, employees, agents, shareholders, partners, affiliates and representatives, from any and all Claims that they may now or hereafter have arising out of, or relating to, the Trial Player participating in the trial under this Agreement. The Trial Player acknowledges that this release is intended to be binding on the Trial Player's heirs, successors and assigns.
Trial Player acknowledges and understands that participation in the trial does not constitute an employment relationship between the Trial Player and the Club and/or NSL, nor does participation in the trial grant to the Trial Player any status other than trialist during the trial period.
The Trial Player is permitted to participate in training sessions with the Club but is not allowed to participate in official NSL matches during the trial period.
This Agreement shall be governed by and construed in accordance with the laws of the Province in which the Trial Player participates in the trial and the laws of Canada applicable therein. Each party to this Agreement irrevocably attorns to and submits to the jurisdiction of the courts of which the Trial Player participates in the trial with respect to any matter arising under or relating to this Agreement.
This Agreement may be executed in several counterparts, by original or PDF or facsimile signature, each of which so executed shall be deemed to be an original and such counterparts together shall be deemed to be the same instrument.
