Growing Up with Type 1 Diabetes
I’m so glad you’re here.
After 25 years as a pediatric endocrinologist, I often wonder how my former patients are doing. I’ve missed you, and I would love to reconnect.
I’m writing Growing Up with Type 1 Diabetes: What People Ask. What You Wonder. What We Wish We’d Known.
Your experiences could help the next generation feel understood. You do not need a perfect story or all the answers. Share only what feels comfortable.
This is an independent personal book project, not a medical-care service. Please do not submit medical records or requests for medical advice.
If you would simply like to reconnect without contributing to the book, email drlynshuebooks@gmail.com. You do not need to complete this questionnaire.
Please confirm your eligibility.

I am 18 years of age or older.
Participation is voluntary. There is no cash payment. Questionnaire participants may request a complimentary digital copy of the finished book if it is published. You may skip story questions. Your decision will not affect any current or future medical care. Participation does not guarantee inclusion in the book. Submitting answers does not authorize publication. Any selected excerpt and identifying details will be sent to you for separate written approval before publication. You may approve, request changes, or decline. You may request withdrawal by emailing drlynshuebooks@gmail.com. Before approving an excerpt, you will be told the withdrawal deadline. Removal from copies already distributed may not be possible after publication. Dr. LynShue will access responses through Jotform. Your contact email will not be published. You will be informed before identifiable responses are shared with any additional editorial helper. Even with a pseudonym, details in a story may make you recognizable. Please avoid naming other patients or sharing private information about other people.
I have read the participation information and voluntarily agree to submit responses for the purpose I selected.
