Some party services may involve the use of cosmetic products, nail polish, adhesives, glitter, lotions, and other beauty products. Please disclose any known allergies, sensitivities, or skin conditions your child may have, including but not limited to reactions to cosmetics, adhesives, fragrances, or skincare products.

I understand that it is my responsibility to disclose any known allergies or sensitivities. If my child has a known allergy or sensitivity, I acknowledge that certain services may be modified or declined for safety reasons.
Skin Sensitivity Acknowledgment
Children's skin can sometimes react differently to cosmetic products. While The Glam Party Truck uses products intended for cosmetic use and applies them in a safe and sanitary manner, mild skin irritation or sensitivity reactions can occasionally occur.
I acknowledge that cosmetic services such as makeup, glitter tattoos, and nail polish are applied externally.
I acknowledge that my child may have a sensitivity that was previously unknown.
I understand that cosmetic products may occasionally cause irritation or allergic reactions, including reactions to ingredients to which my child has no previously known sensitivity. I authorize my child to participate in age-appropriate activities, subject to the allergies and restrictions disclosed above.
I understand that The Glam Party Truck may modify or decline an activity if a disclosed allergy, sensitivity, or other safety concern makes participation inappropriate.
Participation & Safety Acknowledgment
I understand that my child will be participating in activities aboard The Glam Party Truck, a stationary mobile party experience. By signing this form, I acknowledge and agree to the following:

• My child may participate in age-appropriate activities, including mini manicures, mini pedicures, hairstyling, makeup application, crafts, and other activities included in the party package.

• My child must follow all safety rules and instructions provided by The Glam Party Truck coordinators.

• My child must use caution when entering and exiting the truck, including when using the steps, and must be assisted by a party coordinator when necessary.

• My child must remain within designated party areas and may not exit the truck without the assistance or supervision of a party coordinator.

• My child must receive assistance from a party coordinator when entering or exiting the pedicure station.

• My child and accompanying adults must remain away from all restricted areas, including the generator and areas marked by orange safety cones.

I understand that failure to follow safety instructions may result in my child's removal from an activity or the party if necessary to maintain a safe environment.
Emergency Medical Authorization
In the event of an injury, illness, or medical emergency involving my child, I authorize The Glam Party Truck staff to contact 911 or other appropriate emergency services when deemed necessary. I understand that staff will make reasonable efforts to contact me or my designated emergency contact as soon as possible. I authorize staff to share relevant allergy or medical information provided on this form with emergency responders. I understand that The Glam Party Truck staff are not medical professionals and that this authorization does not replace any consent legally required for medical treatment.
Bathroom & Parent Availability Policy
I understand that The Glam Party Truck does not provide restroom facilities for party guests. If my child needs to use the bathroom, a party coordinator will escort my child off the truck and release them to a parent or designated responsible adult. I agree that I or a designated responsible adult will remain reachable and available throughout the party to assist my child with bathroom needs or any other situation requiring parental attention.
Photo & Social Media Consent
Photos and short videos may occasionally be taken during parties for the purpose of celebrating the event and sharing moments from the party experience. These images may be used on The Glam Party Truck's social media pages, website, marketing materials, and promotional content.

Parent/Guardian Acknowledgment & Agreement
I understand that my child is voluntarily participating in party activities at The Glam Party Truck. These services are cosmetic and recreational in nature. By signing below, I agree to release, waive, and hold harmless The Glam Party Truck, its owners, employees, and contractors from any liability, claims, damages, or injuries that may arise from participation in these services, except in cases of gross negligence. I acknowledge that I am the parent or legal guardian of the child listed above and have the authority to consent to their participation.
By signing below, I confirm that:

• I have read and understand the information, policies, and acknowledgments contained in this consent form.

• I have disclosed all known allergies, sensitivities, and relevant medical concerns that may affect my child's participation.

• I authorize my child to participate in The Glam Party Truck activities, subject to the information and restrictions provided in this form.

• I understand that my child must follow all safety instructions provided by party coordinators.
