Public Records Request Form

A public record is a record that is reasonably necessary or appropriate for the Authority to keep in order to maintain an accurate knowledge of our official activities. ARS§39-121.01(b).

Before proceeding, please review our website as many of the Authority's public records are readily available for public consumption.
SUBMITTING YOUR REQUEST
- Limit the request to ONLY those records needed. Ensure the request is SPECIFIC and includes a range of dates for the records.
- Requests that are too broad will be returned for clarification and specificity.
- Any request for documents that are readily available on our website will result in a referral to our website.
- You must disclose if the request is for commercial purposes. If so, a description of the commercial purpose is required.
- Complete and sign the Public Record Request Form.
- Records or parts of records that are not subject to public disclosure under Arizona law will not be disclosed in response to a public records request.
- Records that are subject to disclosure, but which contain information that is private or confidential, will require redaction, meaning that the non-releasable portions of the document will be removed or otherwise obscured so that it cannot be read.
- Under Arizona law, we are not required to compile information in a specific format in response to a public records request. Records will only be provided in established formats.
- We will fulfill your request as promptly as our resources and workload allow.

COMMERCIAL PURPOSE:

In accordance with A.R.S. § 39-121.03 A-D a person requesting records for a commercial purpose must specify the commercial purpose for which the documents being requested will be used.

Please note that there are penalties associated with either using public records or allowing them to be used for a commercial purpose without declaring such intent on this request.
Please describe the records you are requesting. Be as specific as possible (e.g., document titles, dates, departments). If using for COMMERCIAL PURPOSES, you must specify the commercial purpose in your request below.
