Rocky Mountain Equality Youth Field Trip Permission Form to Longmont Climbing Collective

We will be meeting at Longmont Climbing Collective 155 Pinnacle St, Longmont, CO 80504 on Thursday 11/19/2026 at 4:30p and climbing until 6:30p

It is suggested that youth wear comfortable clothing with minimal dangly fabric/ jewelry to avoid getting caught on the rope, holds, or belay system. All jewelry including rings, necklaces, earrings, etc. will be asked to be removed before climbing.  Youth should also bring a water bottle to stay hydrated.

Youth are welcome to bring their own climbing shoes, harness, and chalk if they please, but the gym will provide these items to anyone who needs them.

All facilitators in attendance are belay certified and we will also have a climbing coach with us to assist with harnesses and belaying for maximum climb time.

RSVP and LCC FACILITY WAIVER are required to attend- waiver is located near the end of this form.

 

This field trip is open to youth ages 12-18 y/o and is completely free

Rocky Mountain Equality programs require that parents/guardian sign and return this form to staff before your is allowed to participate in the field trip/activity. I hereby release, and agree to indemnify and hold harmless Boulder Pride dba Rocky Mountain Equality, and any of their respective employees, agents, officers, representative, successors, assign, and insurers from liability for any injury, loss, or damage to my child, or property, whether anticipated or unanticipated, resulting from participation in any activities contemplated by this Agreement.

I realize that going on this trip may involve risks and hazards, including the risk of injury, to my child. I am aware of the risks and hazards inherent in my child's participation and do hereby assume sole responsibility for all such risks and waive all claims against Boulder Pride dba Rocky Mountain Equality and their respective agents, representatives, officers, employees, successors, assigns, board and insurers.

Click HERE to go to the Longmont Climbing Collective and fill out the waiver for the household, including youth under 18. All costs for entry and rental are provided by RMEQ, including harnesses, shoes, etc. You CANNOT climb without a completed waiver.

I hereby grant to any photographer acting on behalf of Rocky Mountain Equality, a Colorado non-profit organization ("Photographer"), the legal representatives and assigns of Photographer, and Rocky Mountain Equality, the irrevocable and unrestricted right to use and publish photographs of me and my children, if any, or in which Ior my children may be included, taken at or in connection with the above-referenced Event, for editorial, trade, advertising, and any other purpose and in any manner and medium; and to alter and composite the same without restriction and without my inspection or approval. I hereby release Photographer, Rocky Mountain Equality, and the legal representatives and assigns of Photographer and Rocky Mountain Equality, from all claims and liability relating to said photographs. I understand that Rocky Mountain Equality is an LGBTQ (Lesbian, Gay, Bisexual, Transgender, and Queer) and allies community services organization and that photographs of me and/or my children may be used in materials and in locations, including online, that are related to lesbian/gay/bisexual/transgender/queen information and services.
