Participant Safety Acknowledgment, Assumption of Risk & Liability Waiver Please read this agreement carefully before signing. Participation in volleyball involves physical activity and carries a risk of injury.
1. Assumption of Risk I understand that participating in volleyball and related activities involves inherent and foreseeable risks, including but not limited to: Slips, falls, collisions with other players, and contact with the volleyball, net, poles, or other equipment. Sprains, strains, muscle injuries, bruises, cuts, fractures, and injuries to the knees, ankles, wrists, fingers, back, or shoulders. Head injuries, concussions, heat-related illness, and other potentially serious injuries. Injuries arising from my own actions, the actions of other participants, or conditions at the facility. I voluntarily choose to participate and knowingly accept the inherent risks associated with this activity.
2. Health and Personal Responsibility I confirm that I am physically able to participate in recreational volleyball and will make my own judgment about whether an activity is appropriate for me. I agree to participate within my abilities, warm up appropriately, wear suitable athletic clothing and footwear, and follow the event rules and safety instructions. I will stop participating and notify the event organizer if I experience pain, dizziness, shortness of breath, or another concerning symptom. I understand that I am responsible for seeking medical advice when appropriate and for my own medical expenses, subject to applicable law and insurance coverage.
3. Safety Rules and Conduct I agree to: Follow instructions from NIYA organizers and gym staff. Use the courts and equipment appropriately. Avoid reckless, aggressive, or intentionally dangerous play. Respect all players, volunteers, and staff. Report unsafe court conditions, damaged equipment, and injuries promptly. NIYA reserves the right to remove a participant from play if their conduct presents a safety concern or they fail to follow event rules.
4. Release and Waiver of Liability To the fullest extent permitted by applicable Illinois law, I release and agree not to hold liable NIYA Wellness and its organizers, officers, volunteers, and event staff for claims arising from my participation in the league, including claims based on ordinary negligence, to the extent such claims may lawfully be released. This provision does not release any liability that cannot legally be waived under applicable law.
5. Personal Belongings I understand that I am responsible for my personal belongings. To the extent permitted by law, NIYA is not responsible for lost, stolen, or damaged personal property.
6. Acknowledgment and Signature By signing below, I confirm that: I am at least 18 years old. I have read and understood this agreement. I understand that this agreement affects my legal rights, including my ability to bring certain claims. I am signing voluntarily and have had the opportunity to ask questions before signing. I agree to follow the league's safety rules and participant expectations.
