Client Consent & Aftercare Agreement

1. Voluntary treatment consent

I understand that completing this form does not establish that a treatment is safe or appropriate for me. Before treatment begins, my esthetician will review relevant information and explain the selected service, its intended benefits, risks, alternatives, and aftercare.

I may ask questions, decline treatment, or request that treatment stop at any time. My esthetician may modify, postpone, or decline a service for safety reasons.

2. Possible reactions & results

I understand that skincare and waxing services may cause redness, tenderness, irritation, swelling, breakouts, allergic reactions, or changes in pigmentation. Depending on the service, additional risks may include skin lifting, abrasions, burns, infection, or scarring.

Reactions can occur despite appropriate precautions. Results vary, and no particular outcome is guaranteed. Esthetic services do not replace medical diagnosis or treatment.

3. Accurate disclosure

I confirm that the information I provide is accurate and complete to the best of my knowledge. I agree to disclose relevant allergies, sensitivities, skin conditions, medications, skincare products, and recent treatments before my service.

I will inform my esthetician of changes before each appointment. I understand that withholding relevant information may prevent my esthetician from identifying risks.

4. Aftercare responsibility

I agree to follow the treatment-specific aftercare instructions provided by my esthetician, including any directions about skincare products, sun exposure, heat, exercise, exfoliation, and touching or picking treated skin.

I understand that failing to follow these instructions may increase the risk of irritation, injury, or other complications. If instructions are unclear, I will ask before proceeding with home care.

5. Undisclosed information & failure to follow instructions

I acknowledge that complications may result from undisclosed information or failure to follow aftercare instructions.

To the extent permitted by applicable law, I agree not to hold V’s Aesthetics LLC or my treating esthetician responsible for harm directly caused by my failure to disclose relevant information requested before treatment or my failure to follow the specific aftercare instructions provided.

This provision does not excuse negligent treatment, unsafe practices, unlawful conduct, or waive rights that cannot legally be waived.

6. Questions or unexpected reactions

I agree to contact V’s Aesthetics promptly about unexpected reactions and seek medical care when appropriate. For a medical emergency, such as difficulty breathing or swelling of the lips or tongue, I will call 911 rather than wait for the studio to respond.
