    D3 Innovations Inc., operating as OneUp Components, collects the information on this form to evaluate your sponsorship application and to contact you about it. Submissions are hosted by Jotform on servers in the EU and are stored in an internal OneUp spreadsheet in Google Workspace, accessed only by our sponsorship team. Your videos, files and screenshots are used only to review your application and will not be published without your permission. We keep applications for 12 months and then delete them. You will not be added to marketing emails.

    Applicants must be 13 or older. If you are under 18, you need approval from a parent or guardian to apply.
