  • SERVICE AGREEMENT SUMMARY. This agreement is between your company and Kadense Jeorling Stroh, an individual ("we" or "us"). 1) Services: we prepare and file the recurring federal and state filings included in your plan (such as UCR, IFTA, Form 2290, MCS-150, BOC-3 coordination, IRP renewal and, on the Full compliance plan, Clearinghouse queries, drug consortium enrollment and driver qualification files) using information you provide. 2) Your part: send the information we request by the date in our reminder, pay government fees directly to the agency, and tell us about changes to trucks, drivers or your address within 7 days. 3) Fees: the monthly per-truck price for your plan, billed monthly; cancel any time before your next billing date. 4) Our guarantee: if we miss a filing deadline after you gave us complete information on time, we reimburse the resulting government penalties and interest up to $1,000 per incident and $2,500 per 12 months, and waive that month's fee. 5) We are a private compliance-filing service, not a government agency, law firm or tax adviser, and do not give legal or tax advice. 6) We use your information only to provide these services.
  • FILING AUTHORIZATION. I authorize Kadense Jeorling Stroh to access FMCSA, Unified Carrier Registration, IRS e-file, IFTA and IRP systems and to prepare and submit filings on behalf of my company using the information I provide. Some agencies require their own authorization forms (for example IRS Form 8821 or a state power of attorney); I will sign those when asked.
