2027 Annunciation Youth Ministry Mission Trip Registration

AYM is headed to Texas! You're invited.
Who: Current high school sophomores, juniors, and seniors, along with parents and adult community members interested in chaperoning. Students and adult chaperones must each complete this registration form.

What: Annunciation Youth Ministry Mission Trip 2027 with YouthWorks

Where: Dallas / Fort Worth, Texas

When: July 9-18, 2027

Why: To grow in friendship, faith, and service to others. Serving those experiencing food insecurity will be a main focus of our time in Texas.

Cost & fundraising
Total cost: $1,200
- Includes transportation, lodging, activity fees, and meals.
- Participants will need extra money for meals while traveling.
- Students are required to complete four fundraising shifts. Each shift earns $50 toward the trip, for a total of $200 toward their trip cost.
- Additional fundraising opportunities will be discussed at the November 30 informational meeting.
- Student participants are required to help at the Stockholders’ Celebration. Date to be announced August 2027.

Spots are limited! Your spot is secured once your completed registration form and $400 deposit have been received. Spots will be filled in that order.

Notes:
- The deposit counts toward your total trip cost and becomes nonrefundable on January 1, 2027.
- Please check your family’s summer plans, camps, sports, and other commitments before registering.

- We’ll review trip details, expectations, fundraising, payments, and questions. All student and adult registrants (with at least one parent) must attend!
First Adult Leader Meeting

7–8 p.m. in the AYM Room

Payments & cancellations
Remaining balances will be due by July 2027, unless other arrangements have been made with Ellie. (To be discussed on at the November 30 informational meeting!)

Medications & Health Conditions

Note: All medication (prescription or over-the-counter) for youth participants must be turned in at check-in and managed by our designated adult leader or nurse, per Catholic Mutual safety policy.
Liability Agreement
In consideration of my child's participation, I agree to indemnify the parish/school and the Archdiocese of St. Paul/Minneapolis from any claims brought against the parish/school/Archdiocese of St. Paul/Minneapolis by myself, my child or others that arises out of any behavior by my child at the event/activity described above. I also agree to pay reasonable attorney's fees or expenses incurred by the parish/school and Archdiocese in defense of such a claim/lawsuit.
Photo & Video Permission
I give permission for photos and videos of my child taken during the mission trip to be shared in Annunciation’s parish and AYM communications, including newsletters, the parish website, and social media.
Participant Policies
Chemical Use/Possession Policy: All youth attending Annunciation Youth Ministries, if found in possession of, buying, selling, distributing or under the influence of mood altering chemicals while on Annunciation grounds, attending any youth group sponsored events, or while riding AYM transportation, this youth will immediately be removed from the event and parents contacted and, if deemed appropriate, local authorities contacted.
Abuse and Harassment Policy: Annunciation Youth Ministries prohibits sexual harassment, physical and/or sexual abuse, whether on or off Annunciation Church premises, or in the course of AYM activities or ministries. AYM will seriously investigate any allegations of such misconduct in accordance with applicable law. Annunciation Church will not retaliate against the reporting person.
Clothing Policy: Concerns have surfaced regarding clothing that is a distraction in youth group. Because of this, we ask that members of AYM recognize that their words, actions and appearance during youth group contribute to the way we present ourselves to our peers and that we are ambassadors of the Annunciation faith community. PLEASE DO NOT WEAR apparel that distracts from the youth group environment, including: tank tops, short shorts, visible undergarments, or any clothing with obscene, vulgar, abusive or discriminatory language or images. This includes apparel that advertises or promotes alcohol, chemicals, tobacco or any other products illegal for use by minors.

The total cost of the 2027 Annunciation Youth Ministry Mission Trip is $1,200 per participant.
This cost includes all transportation, lodging, group meals, program fees, and cultural activities for ten days of travel to and from Dallas Fort Worth, Texas.

To reserve your spot, a $400 deposit is due at the time of registration.
Fundraising opportunities will be discussed at the required Parent Info Meeting on Monday, November 30, 2026 at 7PM in the Church Community Space.

We never want finances to prevent anyone from attending.
