I understand and agree that K9 University, LLC is not liable for damage or injury to any persons as a result of my training on its property, or for participating in this class. I agree to follow training rules and instructor coaching. I give permission to K9 University, LLC to use video images and pictures of myself and my dog which may occasionally be captured during training. I also understand that there is a $50 non-refundable deposit to book into a class. Current Vaccinations Required: DHLPP / Rabies / Bordetella (annually) **Puppies - 2 rounds of DHLPP required **Bordetella and Rabies must be given by 16 weeks of age**If staying in house for Boarding School or Day Camps, we strongly recommend vaccinating with Bordetella every 6 months**
