We will need copies of any previous medical records at the time of your appointment. We offer several options for obtaining and providing previous records:
1) Of course, we can call to obtain your records on your behalf when you arrive for your appointment.
Or you can.....
2) call your previous veterinarian and request that they fax your medical records to Commerce Village Vet at (248) 363-2521.
3) obtain printed a copy from your previous veterinarian and bring them with you to your appointment.
4) email the records to hospital@CommerceVillageVet.com
5) upload a PDF copy of the records using the "upload" box below.
