1. Fields marked with a * red asterisk are required.

2. Please use complete names for all persons.

3. Do not use nicknames.

4. Please use maiden names for all females. Do not use married names!

5. Please enter dates using this example: Jan 01, 2013.

6. There is space for up to 10 children. If you are single and completing a sheet for just yourself, enter your information in the space for Child 1 and then your parent's information in the Husband and Wife fields.

7. If a child is adopted through a second marriage, enter the child(ren) with his/her biological parents. Leave a comment in the comment field as to which surname the child(ren) is/are using.

8. Please complete the Submitter Infomration section. All fields are required except the date.
