Waiver of Liability and Race Agreement
I acknowledge that my participation in UST Clinical Pharmacy Society’s "LACE UP 2013: FEEL THE SPLASH!" – a fund-raising event, is voluntary, thus, I assume my own risk. I grant that the organizing committee is in no way liable of any injury and loss of any personal belongings during the course of the event. I am aware of my fitness and the possible risks of the race. I also give my permission for the free use of my name, photo, video, and/or voice in any media aspect of the event.
Rules and Regulations
§The registration is open to ALL. Encouraged participants are 12-65 years old. Payment is non-refundable.
§Registration and dri-fit shirts are on a first come first serve basis. Registration fee is Php450/person. For pair registration Php800 would be disbursed on a single receipt
§The runner confirms the accuracy of all the information provided on the registration form. Should there be discrepancies found, the organizers have the right to disqualify the involved runner from the event.
§Once registration has been duly processed, there is no registration fee refund for a runner who for whatever reason does not eventually take part in the event. Race slots are non-transferrable
