    Cubbies:  3 & 4 Year Olds

    Sparks: Kindergarten to second grade

    T&T: Third grade thru sixth grade

  • As a parent and/or legal guardian, I do herewith authorize treatment under the direction of any licensed physician of the above named minor in the event of a medical emergency, which in the opinion of the attending physician, may endanger his/her life, cause disfigurement, physical impairment, or undue discomfort if delayed.  This authority is granted only after a reasonable effort has been made to contact parent/guardian by telephone at the numbers given above.

    As a parent/guardian, I assume full responsibility for any cost connected with such treatment and hereby release the church where the child attends AWANA Club and all of the leaders of the club from liability.

    This form is completed and submitted of my own free will with the sole purpose of authorizing emergency medical treatment of my son/daughter in my absence.

  • Uniform Order Form

  • Each Child will need a uniform to display their awards and to be recognized as a club by their dress.  Below is the Uniform information.  When you order a Uniform you will want to get it big enough to last the whole time in that club.  (Ex.  if your child is starting Sparks, you will want to order a vest that will last for 3 years so you will only have to buy the uniform once) 

    The Church will furnish the handbook, patches, awards and items for the AWANA Store.  If a book is lost during the year, parents will be responsible for a new one.  The cost is $10.

     

    Cubbies:

    $25.00 - Uniform Vest (S, M, L, XL); Book Bag

    Sparks:

    $25.00 - Uniform Vest (M-8, L -10, XL (12), XXL -14) ; Book Bag

    T&T:

    $25.00 - Tshirt (youth Size 12, 14, Adult Sizes S, M, L) ; Drawstring Backpack

  • Children recieve their uniform after they completed their entry booklet into their club.  This usually takes 2-3 weeks. 

    You can pay for the uniform when your child is dropped off at AWANA.  If you have any questions about paying for the uniform you can contact Pastor Robbie at info@centervillebaptist.org
