Meyer’s will keep your application in an active file for six (6) months. If your application is not signed, it will not be considered a valid application and will not be processed.
My signature acknowledges receipt of the Background Verification Disclosure and the Authorization and Release ("A&R") forms provided me by Meyer’s separate from this employment application. I submit the signed A&R form along with my completed application. I understand that if I have not completed the application including the A&R form properly, Meyer’s will not consider my request for employment.

request investigative consumer reports on you. Meyer’s will use any such report(s) solely for employment-related purposes.
Any such reports may contain information bearing on your character, general reputation, personal characteristics, and mode of living. The types of information that may be obtained include but are not limited to: social security number verification, criminal records checks, public court records checks, driving records checks, educational records checks, verification of employment positions held, licensing and certification checks, etc. The information contained in these reports may be obtained by private and/or public record sources, including sources identified by you in your job.
The nature and scope of any investigative consumer reports that may be requested is explained above.

I have carefully read and understand this Disclosure and Authorization form. I consent to the release of investigative consumer reports to Meyer’s in conjunction with my job application. I understand that if MEYER’S hires me, my consent will apply throughout my employment to the extent permitted by law, unless I revoke or cancel my consent by sending a signed letter or statement to Meyer’s .
I understand that, to the extent allowed by law, information contained in my job application or otherwise disclosed by me before, during or after my employment, if any, may be utilized for the purpose of obtaining investigative consumer reports.
This Disclosure and Authorization form, in original, faxed, photocopied or electronic form, will be valid for any reports that may be requested by MEYER’S .
