Community Action Promise Award Nomination Form
This award recognizes an individual who has contributed significantly to the Community Action Network, its programs, and activities and exemplifies the true meaning of Community Action. The recipient demonstrates the willingness to go the extra mile to help people in need, respects all people, and is dedicated to the mission and values of Community Action. This award will be given to a current Community Action Agency employee and/or a non-employee who has made significant contributions to the Community Action Network. Non-employees may include CAA board members, volunteers, CSBG State Agency staff, public officials, representatives of a corporation, or community members.
