This form will remain confidential and only be seen by Executive Board Members.
PLEASE REMEMBER that these coaches are all volunteers though. They are parents who have stepped up to coach a group of kids so those kids could play. Often times we have to turn away kids because we're just unable to recruit enough coaches for them all. So please put it all into perspective.
That being said, if you feel anyone is crossing the line with their coaching style, please let us know. But also let us know when you feel a coach is doing well. We use this information in training our coaches, or just giving priority.
