SDM Booking Request Form

Thank you for booking models and photographers through Sweet Dream Models. Make sure that you have looked carefully at the model's comp card or photographer's collage because that is the quality you will receive. Models and photographers have 14 days from the time they receive the package to return photos to SDM. PLEASE send a note with the package of your boutique name, booking through SDM and any special request you had at booking. Packages are required to have tracking on them, insurance is optional. Please allow up to 48 hours for a response from the model. Thanks.
