  • Form Instructions

    Please fill out every portion of this form. Please provide accurate contact information. You will be contacted by an event coordinator within 48 hours of submitting this form. If you cannot be reached by phone or email your registration will be dropped. If you do not get a response from the event coordinator, assume that you are not registered for the event. Please inform your Hagg Lake event coordinator Jack Morby, morby517@aol.com (503-704-1842) if you cannot attend. The HAGG Lake event allows immediate family members to attend and participate in fishing. Please contact Hagg Lake event coordinator Jack Morby if you have additional questions or need further clarification. Pictures may be taken during an event to help with future promotions and to solicit support from sponsors. Registering for the event is an approval to have your picture taken and/or your name listed as a participant for the purposes of soliciting sponsorship and provide media coverage. Please avoid cameras if you do not want a picture taken. The media may be present at our events. You are not required to participate in any media interview if you choose not to. If you do not desire to be photographed or be interviewed by the media, please politely tell them as they approach. Events are intended to be free for veterans and active-duty military participants. Exceptions may occur for items such as fishing licenses.
  • What We Do With Your Information

    Personal information that is gathered on this form by Take a Soldier/Veterans/Active Duty Fishing, its partners, sponsors or any other affiliated organization will only be used for planning purposes for an event. Your information will not be sold, traded or otherwise given away without the express permission of the registrant. By filling out this form, you agree to allow your email address to be added to our newsletter and email databases. You may unsubscribe to either at any time.
  • If you do not receive notification from TASF Portland Chapter within 48 hours after submitting registration form, it is likely your submission failed.  Contact Jack Morby at morby517@aol.com (503) 704-1842, for further assistance.

  • Additional Information

    This section contains additional registration questions for the event you are planning to attend. Please answer all questions. Failure to fill out the form completely and accurately may result in your registration being rejected.
  • Children 12 to 17 must purchase a youth license in advance of the event to fish with you.  The fee is $10 per child and is an annual license.  TASF cannot provide these licenses at the event site. 

  • Who is coming with you?

    Provide first names and include ages of non adult children. (Example: spouse firstname, child's firstname 12, child's firstname 8 etc.)

  • Adult family members, 18 or over, who plan to fish with you who are not active military or veterans must purchase a fishing license to fish in advance of the event.  They are available at sporting goods stores throughout Oregon or online at the Oregon Department of Fish and Wildlife website.

     

  • Please bring life preservers for all non-adult children coming with you.  The Hagg Lake event allows immediate familly members to come with you and fish with you.  Contact your event coordinator for more information.

  • Licenses are free for veterans and active military.  If you do not have an Oregon Fishing License, we will ask for your name, DD214 or Social Security Number, and Date of Birth.  We forward this information to ODFW after the event.  

    Oregon state fishing licenses may be purchased online in advance of your event and may be printed or stored on your smart phone.  Oregon residents will find the annual license a better option if they plan to fish more than once through the remainder of the year.  If your event allows children to fish, an annual license must be purchased in advance for children 12 through 17 years for $10.  

  • Some events may have T-shirts available for the veteran or active duty participants.  Shirt sizes are generally men sizes of small, medium, large, x-large, xx-large, and xxx-large.

    Please provide contact/notification information below in case of an emergency. Please provide an emergency contact of someone who is not attending the event with you. If you are part of a larger group, please list the contact information of the person in charge as either primary or alternate ICE.

  • We do our best to accomodate everyone's needs.  Please list any allergies/dietary restrictions that you or any participant coming with you may have.  Enter None or NA if not applicable.

  • We try to match persons who prefer to fish for trout or warmwater fish to the boat captains fishing species preference.  

  • Events will provide fishing gear for all the various types of fishing (bank or from boat).  If you plan to fish on your own after the event, you are welcome to bring your own equipment.  The Hagg Lake event allows you to bring your own fishing equipment and supplies.

  • The following fishing experience level is not required.  However, it is helpful to match boat captains and volunteers with participants.

  • The complete mailing address is not required.  We do ask that you provide your zip code and city to assist us with any transportation assistance and help us perform statistics on participant demographics. We appreciate a complete mailing address so we can provide event information via mail to you especially if you cannot be contacted by phone or email.

  • PLEASE READ! **Fishing Policy**

    Most of the events that we list are for veterans or active-duty military to be able to go fishing. In the event that your family is invited to the event please understand that they may not be able to go fishing with you on a boat. The Hagg Lake event includes fishing for your immediate family members. Every effort will be made for you and your family to be on the same boat. In most cases we have larger boats that will provide room for everyone in the family, but this is not guaranteed. Bring a lifejacket for each nonadult child that comes with you that plans to be on a boat or on a fishing dock. Nonadult children without a Lifejacket will not be permitted on docks or on a boat. Lifejackets will be available for all adults. Everyone authorized to attend a TASF event may participate in all activities provided by the event organizer and sponsors. However, there is no obligation to attend any activities that you and your family are invited to attend. Every effort will be made to make sure that everyone in your family is cared for. Thank you for choosing to be part of Take a Soldier/Veteran/Active-Duty Military Fishing events. Pictures and video may be taken while you participate that may be placed on our websites or Facebook pages and promotional material. If you don't want your picture used for future promotional materials, please help us help you by avoiding the cameras and let the photographers know that you do not want to participate in future promotional materials. Observers and volunteers may give you things and shake your hand and tell you thank you just for serving our country. Thank you and good luck!   • Waiver of Liability/Hold Harmless Agreement (HHA)

    You agree to be bound by the Take a Soldier/Veteran/Active Duty Fishing Hold Harmless Agreement. A copy of the HHA can be obtained from your Event Coordinator or directly from the TASF website at www.takeasoldierfishing.org. Take a Soldier/Veteran/Active Duty Fishing will not sell, trade or otherwise give away any information that you provide on this form. The information gathered is simply for planning purposes for each event.
  • If no, go to www.takeasoldierfishing.org and click on the hold harmless agreement bar below the signup form.

    Once you have read the hold harmless agreement (HHA), print a copy, sign, and bring with you to the event and give to the check-in volunteer staff.  Extra HHA copies will be available at the registration site if you forget to bring yours.

  • When you submit your entry form you agree to be bound by any and all regulations set forth by TASF, the Game and Wildlife division that controls the area, and any local, State and Federal Laws. If you elect NO as your option, your entry form will be deleted and you will not be signed up for the event.

  • Press "print" to print the form.  Press "submit" to complete the registration. "SUBMIT" must be pressed once.
