Boarding: Please ensure when your dogs come to board that you bring their vaccination card, bed, bowls, collar and lead, enough food and treats for their stay, a tag with a reachable number on it, and poo bags. You dogs may also bring any toys and comforters. We will do all we can to preserve the integrity of these items but can not guarantee their condition.

Walking: Please make sure that you leave your dogs lead and any poo bags and treats which may be required for your dog walker. It is also useful for them to know where the water bowl is to refill as well as a dog towel. 

To the veterinary surgery:

During my absence, Sussex Petsitters, a registered national petsitter, will be caring for my pets and has permission to transport them to your surgery for treatment. I authorise you to treat my pets and will be reponsible for payment to you on my return. Please request a copy of this form and file it with my records if required.

To the Owner/Client:

i hereby give Sussex Petsitters, a registered national petsitter, permission to transport my dogs to the above mentioned veterinary surgery. I understand that Sussex Petsitters assumes no responsibility for the loss of my pets and is released from all liability related to transport, treatment and expense.

This should be someone, not away with you, and who is able to make a decision about surgery or euthanasia
