  • FFA Sponsored Leadership Activities

    Only count leadership activities that have happened since April 1st of the previous calendar year.
  • Career Development Event Participation

    Only count the CDEs that you have participated since April 1st of the previous calendar year.
  • FFA Sponsored Community Service Activities

  • In this section, record you participation in A-C FFA chapter fund raiser activities for this school year. Record the number of mulch bags sold (orders are turned-in) at the time of the submission of the applicaiton.

  • Amanda-Clearcreek School Activities

    In this section, you are to mark the Amanda-Clearcreek school activities in which you have participated for this school year. Only mark a sports season that you completed or are currently competing in. Do NOT mark club sports or community organizations here.
  • Non-school Clubs, Organizations & Activities

  • List the Community Service activities in which you have participated since April 1st of the last school year that were NOT FFA sponsored or organized.

  • "Pin" Awards

    If awarded, you will receive a recognition pin at the chapter award banquet in May. Recognition pins are worn on your FFA Jacket.
  • "Top Member" Awards

    Chose the "Top Member" Award that you're applying for. If chosen, you will receive an award plaque at the chapter award banquet in May.
  • Leadership Camp Award

    This section is used for those applying for the Leadership Camp Award that will send you to the Ohio Leadership Camp. You must be able to attend the camp. The dates are on the chapter calendar (June). There are no alternative dates or replacement activities so check your schedule first.
  • If you would like to apply for the Leadership Conference Award, which will pay for you to attend the 212º Leadership Conference during the upcoming school year, you need to answer the following question in detail to the best of your ability. This award is only open to Freshmen and Sophomores. One to three awards are presented annually. Preference will be given to FFA members who haven't attended the 212º Leadership Conference in the past.

  • If you would like to apply for the Leadership Camp Award, which will pay for you to attend the Ohio Leadership Camp (OLC) during the camp session this June, you need to answer the following question in detail to the best of your ability. This award is only open to Freshmen, Sophomores and Juniors. Preference will be given to FFA members who haven't attended OLC in the past.
