CARDV requires all information gained through service with our organization to be treated confidentially. Discussing any information regarding or pertaining to individuals receiving services with the exception of active volunteers or staff is not permitted.
Confidential information includes:
o Any written, electronic or spoken information and communication between a person seeking or receiving services and any program staff, volunteer, or board member in the course of that relationship;
o Any records or written or electronic information identifying a person to whom services are provided;
o Any information about services provided to an individual.
- A breach of confidentiality is a serious violation of trust and ethical responsibility, as well as a violation of statutes set forth by Missouri law §455.220 RSMo. and federal law 42 U.S.C. §§11383, 13925(b)(2), 10604(d) and 10402(a)(2)(e).
- The location of CARDV is confidential and may not be shared with anyone outside the organization.
- A breach in confidentiality can jeopardize the safety of participants, staff and volunteers, and thus may be a cause for immediate dismissal from service.
I agree not to divulge any confidential information during or after my tenure with CARDV. I also agree to follow all CARDV policies and procedures related to confidentiality.
