I understand that in order to establish greater credibility and support fundraising, Amerisong will use my name and military affiliation as a public endorsement in print, online and otherwise.  I understand that there are no costs, obligations or other responsibilities as a military endorser.

By submitting this form I agree to allow Amerisong to use my name, military affiliation, and rank, publicly, as an endorser.  I grant this permission on the condition that if at any time or for any reason I wish to revoke it, upon my notification, Amerisong will discontinue and remove all future references to me and my endorsement.

Amerisong may use military picture in conjunction with my endorsement at their discretion.
