Please read the following before completing the form below.
- All new students must complete this Enrollment Application and submit it with the $60 enrollment fee. (Enrollment fee is a one-time, non-refundable fee that only enrolls student in the school. It does not include individual course tuition fees or the cost of textbooks. One time means once enrolled in MNSOM, you are enrolled forever and do not pay this again.)
- A Pastor's Recommendation (click here) is required for all students that do not hold Assembly of God Ministry Credentials.
- All students must read the entire Student Handbook (click here) and be familiar with the policies before submitting this form.
- Subject to the approval of the Enrollment Application and Pastor's Recommendation, applicants will be granted access to register for courses.
