By filling out this form, you are committing to provide a Passion 4 Paws pup with a Foster Home until your Foster Dog is adopted... we always say 1 dog at a time. These pups need our help to get ready for their forever family; they may have accidents in your home, they may chew things, they may not know how to sit, they might jump on people... it is not always easy, especially in the beginning. Time, patience, empathy, and guidance are needed. That is a Foster's gift to their foster pup. These pups do not know what they have not been taught; they are all different. This is a commitment to be taken seriously, not lightly heartedly; the last thing these pups need is abandonment, rejection, frustration, and stress conveyed to them. Thank you in advance!

By E-signing this Application below, I agree to not breed animals or sell animals while I am a Foster as that is in direct contradiction of the Rescue's mission and core values. I agree to (if applicable) meet an appropriate standard of care with my own animals with regard to medical care. I agree to maintain an appropriate level of cleanliness with regard to sanitation of the animal's waste and inform Passion 4 Paws immediately should I suspect the dog(s) (either mine or Passion 4 Paws Rescue) have been exposed to any type of contagious disease or pest and work with the Rescue to fully eradicate the problem- this may include fleas, tapeworm, ringworm, ticks, etc.

By signing this Application below, if requested, I agree to allow a Representative from Passion 4 Paws to perform a home inspection and conduct an interview with me and my household members. I understand that Passion 4 Paws will contact my veterinarian, my references, and if applicable, my landlord. I will be informed by a Passion 4 Paws Representative as to whether or not my application has been approved or denied.

I am of legal age, and I confirm have the income and ability to take care of a foster dog. All statements I have made in this application are true, and I understand that this Application may be denied if any of the information supplied above is found to be false.

By e-signing this application below, I understand that it will constitute a legal substitution of my signature, agreeing that I understand and acknowledge that my home, yard, car, and persons may be damaged by the animal I am fostering, and that my animals may pick up parasites or other things from time to time from the foster dog. I understand that in no way is Passion 4 Paws responsible for related expenses. I understand and agree to sign the Foster Contract once presented. I understand Passion 4 Paws rescue assumes no responsibility, nor shall they be held liable for any damage (including death) caused by the Foster dog property, persons, or other pets. Should human medical issues caused by the foster dog occur during fostering, Passion 4 Paws rescue is not liable for any medical bills. I understand that while in my care, Passion 4 Paws cannot control such occurrences, but rather incidents can occur as a result of circumstances while in my care. As a foster, I agree to accept full responsibility for the care of this foster dog, releasing the previous owners, fosters, shelters, as well as holding harmless Passion 4 Paws rescue and its representatives of any liability while under my care. I agree to abide by the information emailed to me about fostering responsibilities and realize that I am committed to each foster dog's welfare while under care within my household. I have read and fully understand that Passion 4 Paws rescue may repossess this dog at any time if the foster expectations are violated or the dog is mistreated.

Lastly, I understand I am committing to foster a Passion 4 Paws dog, and that there is no facility to return a Passion 4 Paws foster dog. I agree and understand that foster dogs are not perfect, and the only reason I would return a foster dog is for serious consideration (IE, does not get along with other animals in the household). I agree if, for any reason, the foster dog needs to be rehomed, you agree to give Passion 4 Paws a minimum of 48 hours' notice.

§ To communicate truthfully and with integrity, both within and outside the organization.

§ To not discriminate against or refuse professional help to anyone based on race, gender, sexual orientation, religion, or national origin.

§ To cultivate an attitude of humility in our work, as we are dedicating our work to something greater than ourselves.

§ To neither malign nor disrespect colleagues or other professionals and to always behave professionally and cordially when representing Passion 4 Paws and when talking to or about other Passion 4 Paws members.

Respect the integrity of the dogs you work with:

§ To treat dogs with the utmost respect, patience, and kindness, ensuring the safety and well-being of the dogs takes place.

§ Reward positive behavior rather than scolding or punishing.

§ To never put the animal in a position to fail (i.e., volatile positions where aggression is promoted or an environment that neglects the safety of the animal).

§ Dogs should never be off their leash outside of a fenced-in area.

Serve as ambassadors for dog adoption:

§ To promote responsible pet ownership and to consider adoption as the first option. If a certain breed is desired, breed-specific rescues should be considered before buying a dog from a reputable breeder.

§ To inform those looking for an animal companion about the benefits of pet adoption. To inform adopters and animal-related community members about existing animal care & control shelter facilities and local rescue groups from which they can adopt.

§ To challenge the status quo without becoming a polarizing voice in the no-kill animal movement.

§ To share information regarding rescue fund-raising opportunities in their area.

Work responsibly and respectfully with animal rescues and shelters:

§ To respect the decisions and requirements of animal control and shelter facilities.

§ To abide by all written contracts and/or verbal agreements entered into with a shelter and/or rescue.

§ To respond to requests from shelters/rescues promptly. If unable to return calls promptly, to make alternative provisions to have calls handled by another member.
