O.W.L. Enrichment Summer Camp

Entering Kindergarten thru 8th grade
DRA "Our Wise Learners" (O.W.L.) Enrichment Camp 2025 is for students entering Kindergarten thru entering 8th grade.

O.W.L. Camp includes Vacation Bible Class, Reading & Math enrichment, as well as Art, STEM, Culinary and Field Trips!!!

Camp Dates: June 2nd thru July 25th

(divided into two sessions)

Registration Fee: $50 per camper

Camp Cost: $150 per week

*Sibling discount offered:

child 2 – 10%; child 3 – 30%;
child 4 and beyond – 50%

Camp Times: 7:30AM to 6:00PM, Monday thru Friday

This Camp will involve daily academic instruction.

Authorization And Waiver To Transport Child

I authorize Deeper Root Academy to transport my minor child in a company Bus or Van, driven by an individual authorized by Deeper Root Academy. I understand my child is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follow the directions provided by the driver and/or staff or volunteer. I understand participation in the identified event is not a requirement for school participation.

I have read, understand, and discussed with my child:

(1) My child will travel in a motor vehicle driven by an adult and my child is to wear their safety belt during travel;

(2) My child is expected to listen to supervising staff/driver, respect staff and other children, the vehicles they ride in, and the people they travel with during the trip;

(3) Riding in a motor vehicle may result in personal injuries or death from wrecks, collisions or acts by riders, other drivers, or objects; and,

(4) My child is to remain in their seat and not be disruptive to the driver of the vehicle.

I recognize participation in this activity, as with any activity involving motor vehicle transportation, my child may risk personal injury or permanent loss. I hereby attest and verify I have been advised of the potential risks, and I have full knowledge of the risks involved in this activity, and I assume any expenses incurred in the event of an accident, illness, or other incapacity,regardless of whether I have authorized such expenses.As a condition for the transportation received, I, for myself, my child, my executors and assigns, further agree to release and forever discharge Deeper Root Academy, and their agents, officers, employees and volunteers from any claim that I might have myself or that I could bring on my child’s behalf with regard to any damages, demands or actions whatsoever, including those based on negligence, in any manner arising out of this transportation.I have read this entire waiver and authorization form, I fully understand its terms and conditions, and I agree to be legally bound by its terms.

Payment Policy

Taking care of your child(ren) is very important to us at Deeper Root Academy. We recognize the responsibility it requires and commit as a team of professionals to meet your family’s needs. Our teachers make sure that your child(ren) have educational activities that are safe and enjoyable throughout the day.

In order to maintain our standard of high quality, we need you to be aware of your responsibility as a parent/guardian.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Deeper Root Academy during the selected camp. In exchange for the acceptance of said child’s candidacy by Deeper Root Academy, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Deeper Root Academy and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Deeper Root Academy including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the events.

Medical Release and Authorization

As Parent and/or Guardian of the named child, I hereby authorize Deeper Root Academy to arrange for medical treatment/examination of my child while in attendance at the academy or on a field trip. It is understood that every effort will be made by the center to contact me, or one of the persons listed at the numbers I have provided, before any medical action is taken.

*I understand my child will be transported by emergency rescue. Choice of hospital may be limited*

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named child. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to Deeper Root Academy and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered camp season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
