I hereby grant permission for my child listed above to participate in the recreational and educational programs sponsored by Robert Fulton Development, Inc. through Noah's Ark Summer Day Camp.  I do hereby give the person(s) operating on its behalf complete permission:

#1 To take my child on any field trip which is planned or supervised by the camp.

#2 To take my child to and from designated program sites in insured vehicles provided.

#3 To administer medication, which I shall provide and which is in accordance with a doctor's prescription.

#4 To transport my child to the above listed physician or hospital and allow them to secure proper treatment at my expense in the event that the legal parent/guardian can't be reached.

 

Children may be dropped-off as early as 7:00am at no additional charge. After-Care hours are 3:30pm - 5:30pm for an additional cost of $50.00 per week, per child. A discount of $10 for each additional child up to four children in a family applies:

2nd child = $40 per week

3rd child = $30 per week

4th child = $20 per week;

the total cost for a family with 4 children = $140 per week.

 

Any child left after 5:30 p.m. will be charged $5.00 per 15 minutes late.

The following person(s) are authorized to pick up my child (other than parents) from camp.

Since your child's last health exam, has he/she had:

Specify causal agent and nature of reactions - e.g. penicillin causes hives

Noah's Ark Summer Day Camp Information-to-Note

Camp Tuition

The weekly fee is $105.00, which is due on the Monday of each week. For families with 2 or more children, a discount of $5.00 for each additional child up to four children in a family applies. For example, for a family with 4 siblings the weekly fees will be as follows:

1st child = $105.00                                                             Total: $105.00

2nd child = $105.00 + $100.00                                         Total: $205.00

3rd child = $105.00 + $100.00 + $95.00                         Total: $300.00

4th child = $105.00 + $100.00 + $95.00 + $90.00         Total: $390.00

 

• Noah’s Ark serves children 5 to 13 years of age or Pre-K thru 8th grade.
• Camp hours are 8:00am to 3:30pm Monday through Friday beginning Monday,July 31, 2023 and ending Friday, August 18, 2023. 
• The program will include some field trips, outdoor games, music, movies, arts & crafts, and other educational activities.
• Minimal additional costs may be required for field trips and/or special activities.
• There will be 2 opportunities to attend Parent Orientation. At this meeting parents will have the opportunity to get an overview of this year’s activities, review the Parent Handout, and meet camp staff.

By signing, you agree that you are the parent or legal guardian of the above named camper and are over the age of 18.  

In case of medical emergency or general medical care, I give consent for medical treatment for the above named camper by authorized personnel.  I understand that the above named camper will only be released to the names listed above; an update may be done at orientation.  I certify that my child has my permission to attend camp and participate in all activites. I authorize Noah's Ark Summer Day Camp to use my camper's picture, testimony, and video in any promotional material associated with Noah's Ark Summer Day Camp (web, print or media).

having been made aware of the activities the campers will be doing, I hereby consent to the camper’s participation at the Noah’s Ark Summer Day Camp. I voluntarily release and forever discharge the Noah’s Ark Summer Day Camp and its affiliates from any and all liability, claims, actions, or rights of action that is in any way related to the camper’s participation in the camp activities. I further agree not to sue, assert or otherwise maintain any claim or cause of action against Noah’s Ark Summer Camp arising from the camper’s being filmed or photographed during the summer camp and give permission to use their image for any type of publication.

In case of emergency, I understand every effort will be made to contact parents or person listed as emergency contact. However, if parents or emergency contact cannot be reached, I hereby give Noah’s Ark Summer Camp staff permission to act on my behalf in seeking and administering medical treatment to my child. I agree to release Noah’s Ark Summer Camp from any liability in acting on my behalf in this regard and rendering such medical treatment. I assume the risk and financial responsibility for any injury resulting from my child participation in Noah’s Ark Summer Camp activities.
