This form allows the Chapter to nominate a Member of Phi Sigma Pi for a Distinguished Service Key, to be awarded by the National Council. Distinguished Service Key guidelines can be found in the National Constitution.
Historically, a Distinguished Service Key has been awarded to a Brother whose contributions nationally or on behalf of the National Fraternity have been outstanding. Chapters who wish to recognize individuals whose contributions are limited to a Chapter are encouraged to consider awarding them a Chapter Service Key.

The nominating Chapter must include a 200 to 500 word detailed essay describing why the nominee should be awarded the Distinguished Service Key. In order to nominate a Member, the Chapter must submit the nomination to the National Headquarters by June 1st.
Alternatively, an individual member may submit a recommendation to the National Council. The National Council members will review the submission and an National Council member may decide to bring that recommendation forth to the rest of the Council as a nomination.
Please note: In this scenario, since the recommendation is not a nomination as outlined by the National Constitution, the National Office will not respond to recommendations.
