I authorize Avalex to evaluate the units listed above with the understanding that:

1. Warranty Repairs: Avalex will cover the repair costs and return shipping on units less than one year old. Free shipping is only applicable when shipping UPS Ground on returning a warranted repair. Other shipping methods will be at the Customer's expense.

2. Non-Warranty Repairs: Average repair costs for units older than one year are as follows:

$900.00 for Map Systems, $1,200.00 for Displays, $500.00 for DVRs, and $500.00 for Miscellaneous equipment.

If no fault is found, customer will be charged for testing and troubleshooting labor costs, plus shipping. Labor is $96/hour. The average turn around time for a repair is approximately four to six weeks.

If the ‘Request for Quote Prior to Repair’ option is waived above, I authorize Avalex to make the necessary repairs and bill accordingly once the repair is completed.

When applicable, expediting the repair of an asset is an available option at 25% of the total repair costs (plus shipping). For more information, please contact the Repair Department.
