Terms, Conditions and Release of Liability
Thank you for your interest in working/volunteering for the youth basketball event, sponsored and produced by International Ballplayers United, Inc. (iBall United). A volunteer certificate is available after the event upon request.
Data Protection
Volunteers play a vital role in the success of iBall United and we appreciate you giving your time and talent to achieve success. The information you provide will be stored in confidence under the provisions of the Data Protection Act. Your completed form will be held securely and confidentially. Only authorized staff will have access to your information.
Release of Liability
In consideration volunteering/working at the youth basketball event, I release from liability and waive my right to sue International Ballplayers United, Inc., their employees, officers, volunteers and agents from any and all claims, including claims of the iBall Uniuted negligence, resulting in any physical injury, illness (including death) or economic loss I may suffer or which may result from my participation in this Activity, travel to and from the Activity (including air travel), or any events incidental to this Activity.

By consenting to volunteer/work here you give iBall and its representatives permission to take, use and release photos or video of you and your property in connection with the above-identified subject.
PA Child Abuse & Clearance
iBall United requires all volunteers to confirm that their criminal records are completely clear of child abuse infractions. Your submission of this form confirm you have never been charged or convicted of an abuse or neglegance related infraction. Per Pennsylvania law, it is required that all persons having direct contact with school children obtain background checks and clearances. In the best interest of the safety and security of our students, iBall United recommends the submission of these clearances prior to volunteering.
Valid Clearances are within one year of the submission date.
Instructions to receive clearances
State Clearance- Click the control number to view your results. Click the "Certification Form" link in the center of the page to view, save and print your official certificate.
PA Child Abuse History Certification (electronic) - Click "Create Individual Account". The Keystone Key is a username you create. Follow the site's step-by-step instructions for creating a password. Be sure to keep a record of your Keystone Key and Password, as you will need them to access your results. You will receive an email from the site when your results are available, within 14 days of submission. Or PA Child Abuse History Clearance (paper form).
FBI Fingerprinting (2 Options)
If you have been a Pennsylvania resident for the past 10 consecutive years, you may submit the Volunteer Affirmation in place of fingerprinting.
If you have been a Pennsylvania resident fewer than 10 consecutive years, you must complete the fingerprinting process. Enroll at IdentoGo and make an appointment for digital fingerprinting at a location of your choice.
