Donation Request Form
CMOM often receives in excess of 100 applications for donations every month, and can only fulfill half a dozen of these requests. If your request is fulfilled, CMOM will send admit-one passes or membership paperwork in the mail. No follow-up calls or Emails please.
Organizations must meet the following guidelines in order to be considered for donations:
1. The fundraiser must benefit children in the tri-state area.
2. Donation requests will be entered into a raffle and drawn on the month prior to the beginning of the month in which the fundraiser will take place. All donation requests must thus be submitted at least two months in advance i.e. by the beginning of February for events taking place in April.
3. Organizations who have previously received a donation from CMOM may not submit another donation request within three years of the award date.
If your donation request meets the guidelines above and is selected, you receive a donation package via mail.
