Media Release and Consent Form
Qatar Foundation International ("QFI") periodically photographs and videotapes individuals who participate in activities sponsored by QFI, or relating to events, programs, and/or organizations associated with QFI. Qatar Foundation ("QF") is the sole member of QFI and provides funding to QFI so that QFI can carry out its mission. QFI and QF will use these images for promotional or educational purposes to reflect its programs and mission.

In consideration of the opportunities QFI provides to you and/or your child, we kindly request that you countersign a copy of this letter giving QFI and QF permission to use the photographic or videotaped images of you or your child for the foregoing purposes.

Purpose of Declaration
The Recordings will be used by QFI for the following purposes:
- For internal educational purposes of our employees, including the provision in our intranet;
- In order to advertise the work of QFI in general (including without limitation the promotion and publicity relating to QFI’s 10th anniversary)
- For internal monitoring and evaluation processes (numbers of participants, demographics, etc.).
The Recordings will be used for the abovementioned purposes in the following media:
- On the Internet on the websites of QFI
- On the Internet on the following social networks: Facebook, Twitter, LinkedIn, Instagram, YouTube.
- In printed matter (i.e. flyers, brochures, books.
- In newsletters distributed via e-mail.
QFI reserves the right not to make use of any Recordings or to use them only for some of the purposes or in some of the media listed above. In general, the Recordings will be used globally. As far as information is globally accessible, it may be found with search engines and connected to other information, from which personality profiles may be generated. Information made available on the Internet, including photos and videos, may easily be copied and disseminated. There are specialized archiving services, whose purpose is to permanently document the particular state of the website on certain fixed dates. This may result in published information being available on the internet even after its deletion from the original page.

The Recordings will be stored and processed electronically. The processing includes editing the Recordings for the abovementioned purposes, including the use of parts and extracts on the Recordings. QFI emphasizes that upon publishing the Recordings on the Internet, they may be accessed globally and may be further disseminated.

Since your rights regarding your spoken word and your own image are affected, we request your consent for the production and abovementioned use of the Recordings.

Signing this declaration of consent is voluntary and it becomes effective with your signature. You may at any time revoke your consent with effect for the future, in which case:
- Recordings published online: Recordings primarily showing you will be removed from our Internet presence within a period of 2 weeks. As far as you are depicted with other individuals in the Recordings, the Recording does not have to be removed, it is sufficient that you are made unrecognizable in the Recordings (i.e. by means of pixelation) within 2 weeks. If you are depicted together with other individuals and QFI elects not to make use of the possibility of pixilation, but elects to directly replace it with new material, the Recording must be replaced within one month.
- Recordings published in printed medias: Your recording will not be used in new printed matters, meaning matters printed after the date you informed us about the withdrawal of your consent.
