Upload Change Form

Change Form Upload instructions:
1. (*) = Required
2. Click the "Upload A File" button
3. Navigate to the location of your change form on your device
4. Click the "Open" button
5. Once the form has been selected click "Submit Change Form"
6. BE PATIENT!!! This may take a few minutes depending on the size of your file and your internet connection speed.
7. When the form is successfully uploaded you will be given a confirmation.
