Parent/Guardian ID Request Form

If you are the Parent or Guardian of a BFS student, and your ID is lost or no longer works please fill out the following form. Once the form is completed and submitted, the Brooklyn Friends Tech Dept. will print out an ID card which will be available for pickup at the receptionist's desk at 375 Pearl Street.

In order to process your request, we need a photo of the parent/guardian whose name will appear on the card. You may either submit a jpg image, take a photo with your computer, or have your picture taken at the BFS 375 Pearl Street security booth. Please note, submitting a photo with this form will speed up the processing, and we will be able to give you your card sooner.

If you choose to submit a picture, please make sure it is well lit, that the subject is facing the camera, You can see the subject's head and shoulders, and that they are not wearing anything that obscures their face (i.e. sunglasses, hats, masks).
