This form is to request pickup of Shredding Orders that have already been requested and issued an Order #. To find your order number refer to the automated e-mail you received. If you cannot find your Order # please send an e-mail to shredding@colostate.edu to request it. If you have not submitted the inital request, please visit www.shredding.colostate.edu to submit your initial request.

Please DO NOT fill out this form until your shred containers are ready for pick up OR you have labeled your boxes with GREEN SHRED labels. In order for your shred containers to be picked up, the containers must be in an ADA accessible location. Filling out this form prior to receiving the labels or before your containers are ready for pick up could cause delays. Thank you!!
