This form is used for book bag, homework shelf reserves, and bibliography requests. If you would like to have a librarian visit your school or event, please contact our Outreach Services Department at (630) 834-1164 ext. 103. PLEASE ALLOW 5 BUSINESS DAYS TO PREPARE EDUCATOR BOOK BAG REQUESTS. Educator Book Bags are limited to 20 items per topic.
