  • CiViL is a program that builds the Character, Values, and Leadership skills students need for success! Your student can be part of weekly small-group discussions – CiViL Groups; engage in monthly service-learning projects – CiViL Service; and explore exciting enrichment opportunities, including eye-opening college visits – CiViL Liberties. CiViL builds resilience, develops empathy, and charts a path toward success in a supportive and empowering environment. CiViL is a Character, Values, and Leadership development initiative of Touchstone Youth Resource Services, Inc.

    Parent Permission for your student to participate is required by the school per an order by the State of TN. This information will be kept private and will not be shared by anyone outside of Touchstone. Please fill out and sign the form below:

  • Student Club/Organization Parent Consent Form

    In connection with the legislation concerning extracurricular activities, parents/guardians are required to provide consent for their student to participate in student clubs/organizations.

    Section 49-6-1031 - Extracurricular activities (a) Each school shall notify the parents or legal guardians of all clubs and organizations available to students attending such school by prominently displaying the information in the school's student handbook, or other standard or policy guidebook that contains the policies and procedures of the school and is distributed annually. The list shall include: (1) The names of the clubs and organizations, including any abbreviations or acronyms; (2) The mission and purpose of the clubs and organizations; (3) All financial requirements associated with membership in the club or organization; and (4) Notification of the option set forth in subsection (b) to prohibit a student from participating in any club or organization. (b) (1) A school shall not allow a minor student to become a member of a club or organization, or allow a minor student to participate in any activity of a club or organization, unless the student's parent or legal guardian first provides consent to the student's membership or participation in a dated, written consent. (2) As used in this subsection (b), "minor student" means a student who is not eighteen (18) years of age or older.

    My signature below acknowledges that:

    I agree to allow my student to participate in CiViL Groups - on a weekly basis, and CiViL Service projects and CiViL Liberties events – as they are scheduled. I also understand that my student’s participation is a privilege, not a right, and it is at the discretion of the CiViL directors.

    I release CiViL of Touchstone and all employees, volunteers, agents and representatives thereof, from all liability for injuries, including death, to the student named above, and for all claims, loss, or damages resulting from the student’s participation in any CiViL activity, including being transported to and from meetings, service projects or other events.

    I assume full responsibility for all risk to the student while they are participating in any CiViL of Touchstone activity, and I assume full responsibility for any loss or damage caused by the student’s behavior while participating in CiViL of Touchstone activity.

    I authorize CiViL of Touchstone to obtain medical aid for my child in case of emergency
