Answer if the report reveals a criminal record or other information you believe may negatively impact a decision (SecurTest does not make hiring or employment decisions, as this is the sole responsibility of the user of your background report).

Explain why you should be considered for employment or placement.  Explain in detail any information you believe will help the user of your report understand the person you are today and that will place you in the most favorable light possible.  This information will be used in the Individual Assessment.  Each background report is carefully reviewed on an individual basis, as the iReviewNow process allows the user to consider your responses to make a fully-informed decision.  The process considers the nature and severity of any negative information, how long ago the incidents occurred, and what you have done to improve yourself, known as self-rehabilitation.  Examples include: what you have done to improve yourself, lessons learned, and steps you have taken to better yourself.

I have carefully reviewed my answers and certify that it is true and correct. By clicking on "Submit" I hereby indemnify and forever hold harmless iReviewNow, LLC and SecurTest, Inc., their assigns, licensees, and affiliates, and User(s) of my report(s) and iReviewNow responses, and the employer or entity I authorized to investigate my background from any and all claims arising out of validating this information with the answers I have given. My answers will become part of my report/record to help clarify, correct or validate my biographical, background or consumer information, which is exclusively owned by iReviewNow LLC and SecurTest, Inc. or their assigns. I have freely and voluntarily elected or chosen to use iReviewNow as the sole and single method for disputing, commenting, or correcting any negative information in my background or consumer report. I have used the benefits of iReviewNow as my personal assurance that the report about me is complete, accurate, up-to-date, and certify and affirm that iReviewNow, LLC and SecurTest, Inc. have used every reasonable steps to ensure the accuracy of my report.

I hereby authorize SecurTest, or the background screening provider and user of the report to transmit my background or consumer report to iReviewNow, LLC and hold each harmless from any loss, liability, damage, cost or expense (including reasonable attorney fees and costs) arising out of any claims or suits that may be brought or made against them by me, others acting in my behalf, or any other party.

I further agree that any dispute arising between iReviewNow, LLC, SecurTest, Inc. or any other party described herein shall be resolved through binding arbitration conducted by the American Arbitration Association ("AAA"). The conduct of the arbitration shall be governed by the AAA’s rules for commercial arbitration. A three-member panel selected from the AAA’s panel of commercial arbitrators shall hear the arbitration. The selection process will be governed by the AAA’s rules for commercial arbitration. The arbitration shall be conducted at a location solely determined by iReviewNow, LLC. I hereby expressly waive all access to court, including the waiver of any right to seek an injunction, other than to enforce an award already made by an arbitration panel. I agree that I am solely responsible for payment of all arbitration costs and attorney fees if I file a claim for arbitration, as I have received benefits by my use of iReviewNow. Where the Arbitration panel agrees with me, in whole or in part, that my damages and award shall not exceed $25.00 due to the substantial benefits already received by me for use of iReviewNow. Where the panel awards or decides in the favor of iReviewNow, LLC, SecurTest, Inc., or any named party I have named, I understand and agree that I shall be responsible for each party’s costs and attorney fees.

I affirm and agree that iReviewNow is not a consumer reporting agency, as defined by the Fair Credit Reporting Act, or other federal or state legal definitions. I agree and affirm that iReviewNow is a patented compliance and quality assurance tool that benefits me in viewing my background or consumer report and alert the user or consumer reporting agency of any disputes or other information afforded by iReviewNow.

SecurTest is the background-screening provider that performed the background investigation. SecurTest does not decide my eligibility for employment, access control, clearance, security clearance, volunteerism privileges, employment, application for employment, acceptance in any government facility entry program, or for any other purpose. iReviewNow, LLC is the patented solution that allows me and authorized users of my background or consumer report(s) to review the report, correct any misinformation, dispute any information, provide context about my information, and provide positive feedback about positive steps toward self-improvement or rehabilitation.
