The F-1 Annual Vacation Request Form must be submitted before the add/drop deadline of each term. International Students on F-1 status are eligible to request annual vacation only if you have registered for three, full-time (consecutive) trimesters in one academic year while on F-1 status each trimester

If you are not registering for courses OR Internship during your Annual Vacation, you must submit an additional form with University with the Registrar's Office. Submit this Leave of Absence form. Student must be on Good Academic Standing to qualify for Leave of Absence. If the Registrar's Office denies your LOA, you must register for courses during your Annual Vacation.
