TD1 form is something we need on file to authorize what level of withholdings we do for taxes etc. and each employer should ask you for one each year, here is the link, just select the PDF fillable/savable version

For most people you simply fill in the top section personal information, put the Basic Personal amount (from box 1) in box 13 at the bottom of page 1 and sign the bottom of page 2, however please read it to see if any other deductions apply and if you do claim more than the basic amount then you need to complete a provincial form as well
