I. TOUR DETAILS:
This tour provides stays in local villiages/hotels, visits to historic/tourist sites, games/challenges and an optional safari or luxury version. I understand that participation in tour activities requires full payment and abiding by the rules of conduct within this agreement.

II. CONDUCT AND ADVISORY:
I acknowledge that this trip is not recommended for pregnant, nursing, disabled, or the elderly; or for people with health concerns that impede ambulation, endurance, or mental health. I understand that I am only entitled to three (3) additional guests for a total of four persons for the stay. Additional guests add to the grand total and s/he must fill out a registration formand provide identification (ID/Passport). Safari and Luxury levels add an additional cost per guest respectively. I also understand there is a nonrefundable 10-11% service fee for the trip. I acknowledge my responsibility to arrive on time at rest stops, pickup locations and other rendezvous points arranged by the Consultant/Tour. I understand that the consultant is not responsible for airline delays and service delivery of local merchants/providers/partners or my interactions with local law enforcement or immigration officials. I acknowledge that guests not listed on registration are not entitled to entry or participation in activities or premises. Guests consent to abide by tour rules, immigration laws and all local and national regulations of the country where the tour is hosted.. The participant and guests acknowledge that weapons, illegal drugs, contraband, public nudity, child pornography, animals and banned/illegal substances and activities are not permitted on this trip. I consent to village quiet hours begining at 8:00 p.m. I acknowledge that children under the age of 13 are not permitted on this experience.

III. PAYMENT AND SERVICE DELIVERY:
I understand and consent to pay the grand total to participate in the selected tour. I am entitled to a refund, less fees if I cancel not less than 72 hours before the trip. Full refunds less processing fees are available for reversals within 48 hours. No shows do not receive refunds and are considered to forfeit the entire trip. A no-show is considered anyone who fails to notify the consultant 72 hours before the commencement of the tour during business hours (9 AM-5 PM UTC+2). I understand that the Consultant is not responsible for any loss or injury I incur on the trip and I consent to sign all waivers and consent agreements with the exception of photographic releases for minors. Photographic releases for minors remain at my full consent, provided I am the legal guardian of the participating minor(s). I understand that I am traveling to a developing country and as such accommodations and infrastructure may not always be adequate. I acknowledge that if I choose not to communicate my intention to cancel during the entire period of the trip, I shall forfeit the entire trip and any entitlement to refunds or payments.

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH the Contractor (Three2One Tours/Winston Media Concepts).including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or
entities being released, from dangerous or defective equipment or
property owned, maintained, or controlled by them, or because of
their possible liability without fault.
I certify that I am physically fit, have sufficiently prepared or trained
for participation in this activity, and have not been advised to not
participate by a qualified medical professional. I certify that there are
no health-related reasons or problems which preclude my
participation in this activity.
I acknowledge that this Accident Waiver and Release of Liability
Form will be used by the event holders, sponsors, and organizers of
the activity in which I may participate, and that it will govern my
actions and responsibilities at said activity.
In consideration of my application and permitting me to participate in
this activity, I hereby take action for myself, my executors,
administrators, heirs, next of kin, successors, and assigns as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability,
including but not limited to, liability arising from the negligence or fault
of the entities or persons released, for my death, disability, personal
injury, property damage, property theft, or actions of any kind which
may hereafter occur to me including my traveling to and from this
activity, the contractor and/or their directors, officers, employees, volunteers,
representatives, and agents, and the activity holders, sponsors, and
volunteers;
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE
the entities or persons mentioned in this paragraph from any and all
liabilities or claims made as a result of participation in this activity,
whether caused by the negligence of release or otherwise.
I acknowledge that they are NOT responsible for the errors,
omissions, acts, or failures to act of any party or entity conducting a
specific activity on their behalf.
I acknowledge that this activity may involve a test of a person's
physical and mental limits and carries with it the potential for death,
serious injury, and property loss. The risks include, but are not limited
to, those caused by terrain, facilities, temperature, weather, condition
of participants, equipment, vehicular traffic, lack of hydration, and
actions of other people including, but not limited to, participants,
volunteers, monitors, and/or producers of the activity. These risks are
not only inherent to participants, but are also present for volunteers.
I hereby consent to receive medical treatment which may be deemed
advisable in the event of injury, accident, and/or illness during this
activity.
I understand while participating in this activity, I may be
photographed. I agree to allow my photo, video, or film likeness to be
used for any legitimate purpose by the activity holders, producers,
sponsors, organizers, and assigns.
The Accident Waiver and Release of Liability Form shall be construed
broadly to provide a release and waiver to the maximum extent
permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY
UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A
RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY
OWN FREE WILL.

CONSULTANT/TOUR RELEASE OF LIABILITY

I. THE PARTIES. This Informed Consent and Assumption of Risk and Release of Liability
(“Release”) is made on the Effective Date of this Agreement and entered between: Client and Consultant/Tour Guide.
II. TOUR GUIDE/OPERATOR/CONSULTANT. The Consultant is affiliated with a tour under the name of Three2One Tours/Winston Media Concepts Sole Proprietorship. This Release will also cover and indemnify the Tour Consultant/Company/Partners. -
III. INFORMED CONSENT. The Client acknowledges, certifies, and accepts the following: 1) Physical Condition. That they are of adequate
physical condition to participate in exercise despite any current medical conditions they may possess. 2) Assumption of Risk. That they assume the risk of physical injury from any advice, instruction, or action conducted during or as a result of a Participation in activities with Consultant 3) Reporting Discomfort. That any discomfort, distress, or uncomfortable feelings will be immediately brought to the attention of the Tour Consultant/Partners.4) Indemnification. That they will NOT hold the Consultant/Company or its employer, affiliates, agents, or any other entity or individual connected to them, either directly or indirectly, liable for any result from the Activities. 5) Responsibility. That they, the Client, assume all responsibility for a Client’s participation in the Sessions/Tours/Activities.

I (“Releasor”) grant permission and consent to:
Winston Media Concepts/Lela Winston/Three2One Tours (“Releasee”) for the use of the following photograph(s) as
identified below for presentation under any legal condition, including but not limited to publicity,
copyright purposes, illustration, advertising, and web content: public photos of tour and event activities.
I. Payment (check one)
I understand that there shall be no payment for this release.

I understand that with my authorization below the photograph(s) may never be
revoked.
Images will be stored in a secure location and only authorized
staff will have access to them. They will be kept as long as they are relevant and after that time destroyed or archived.
We, the Releasor and Releasee, have understand and agree to the aforementioned terms and
conditions.
