FESTIVAL ARTIST PERFORMANCE FORM & LIABILITY WAIVER
The Artist acknowledges and agrees that participation in the Festival and all related activities involves inherent and heightened risks, including but not limited to personal injury, death, illness, property loss or damage, crowd behavior, weather conditions, equipment or stage failure, acts of third parties, and the Artist’s own actions, including but not limited to stage diving, jumping from or off the stage, entering the audience, encouraging crowd interaction, or similar conduct. The Artist voluntarily assumes all such risks and hereby irrevocably releases, waives, and discharges the Festival, Promoter, Venue, sponsors, and their respective owners, officers, employees, agents, contractors, and affiliates from any and all claims.
FESTIVAL ARTIST CODE OF CONDUCT
The Artist agrees to conduct themselves professionally and comply with all Festival rules and safety requirements.
ACKNOWLEDGMENT
Participation constitutes acceptance of all Festival Artist terms.

The 4Fo7Selm Music Festival is a family oriented event, which is kid friendly. Artist please be aware of the language and content you deliver. (excessive language and behavior) The event is prohibited of alcohol and drugs on the property, so please be considerate of that when on the property. (any questions on this feel free to ask)
IF ANY ARTISTS HAVE A SITUATION AT THE FESTIVAL PLEASE HEAD TO THE F.I.T. (THE FESTIVAL INFORMATION TENT) AND LET SECURITY AND STAFF KNOW ABOUT THE SITUATION. PLEASE DO NOT HANDLE ANYTHING YOURSELF.
