I, the undersigned “Releasor/Releasee”, have voluntarily chosen to utilize services such as but not limited to training, grooming and/or doggy day care with Claircrest, LLC, dba 4 Paws (InSync, 4 Love, Nsync As a condition of participating in 4 Paws services and accessing PN Management dba Camp Claircrest, I legally bind and grant 4 Paws and/or “Provider” for me and my dog(s) and agree to this waiver of liability, general release, and covenant not to sue as follows: I authorize 4 Paws to assist in the care, training and/or grooming of my dog(s) and understand I am accepting responsibility for my dog(s) should they cause damage to the facilities, other dog(s) they are exposed to, and to follow any instructions specified. I understand day care, training and/or grooming comes with inherit risks and I hereby agree and covenant on behalf of Releasor affiliates, not to file any claim, lawsuit or other proceeding, whether judicial or administrative, against the Releasees for any Claim. Upon booking services the fee is due.  If fee is unpaid one (1) day after services are rendered a $20 per day fee will apply.  Customer agrees to all collection fees included in final payment.  Releasor shall indemnify, defend and hold harmless each of the Releasees against and reimburse any one or more of them for any and all claims and liabilities directly or indirectly arising out of any lawsuit, claim, or other proceeding filed against one of more of the Releasees by Releasor or any Releasor Affiliate. Under no circumstances will the Releasees be required to seek recovery from any insurer or third party or otherwise mitigate their losses and expenses, in order to maintain and recover fully a claim against

I hereby agree and covenant, on behalf of myself, my heirs, my personal representatives, my executors, assigns and insurers to release, acquit, hold harmless, waive and forever discharge 4 Paws, Camp Claircrest, the specific Instructor(s), Nickie Hertzog and their respective owners, officers, directors, members, managers, employees, contractors and agents (collectively “releasees”) from any and all claims, demands, damages, debts, costs, fees, obligations, loss, expenses, compensation, actions or causes of actions whatsoever of every nature, character, and description whether known, unknown, discovered, undiscovered, suspected or unsuspected (collectively “claims”) including but not limited to claims arising on account of or in any way pertaining to this release for services or any release negligence or the negligence of any third party in connection with the services.

I hereby give 4 Paws, Camp Claircrest, Nickie Hertzog and their legal representatives and assign, the right and permission to publish, without charge, photographs taken during services. These photographs may be used in publications, including electronic publications, or in audiovisual presentations, promotional literature, advertising, or in other similar ways

Disclaimer: Dogs with aggression issues will not be allowed to continue per the program’s discretion and dogs must be able to follow basic commands. No refunds if the day care is unattended or for any other reason. Services will attempt to be rescheduled for weather, acts of God or other unforeseen issues if possible when cancellation is deemed necessary by the instructors; however, there are no guarantees or refunds. Specific instructors are not guaranteed.

All vaccinations must be up to date and proof of such is required to be supplied. In addition, if my dog(s) is/are ill or has any communicable diseases or conditions, I will not bring the dog(s) to class. I hereby agree to follow the rules and guidelines at Camp Claircrest and if I or my dog(s) do not follow the guidelines per Camp Claircrest I may be asked to terminate my class involvement/enrollment without a refund.

I have read and completely understand this document, and I hereby agree to the release of claims and waiver of liability set forth above (must be eighteen (18) years or older):

Please note: Your dog should be brought to each class unless the female is in season or a health issue occurs, in which event observation arrangements may be made and/or with the potential for a make-up class. In addition, bring a 6’ leash, plenty of training treats, buckle collar or other training collar and anything else listed on the particular class description. Copyright 2021
