PARENTS AND SENIOR: READ THOROUGHLY BEFORE SUBMITTING. By clicking submit, I understand that if I am chosen as a AMC Model, I will be required to use April Moore Creations, LLC as my senior photographer and will contractually agree to not use any other photographers for the duration of the program. I also understand that this application will be reviewed by April Moore Creations and is not a guaranteed acceptance into the Model Team. In addition, I understand that, if chosen, I am required to pay the cost of a senior session, but will have the opportunity to earn money towards products through referrals.

I agree to meet the obligations I am committing to, such as enthusiastically representing April Moore Creations and keeping the appointments that I schedule. I further agree that I will not represent any other photography studio or photographer from acceptance until end 2021. (this excludes school events/sports/clubs etc.)

April Moore Creations may use the portraits created of me for promotion, advertising, competitions, and display purposes. I understand the images will be selected as needed by April Moore Creations.

I also agree to the Copyright rules, that I may not copy, print, or edit images created by April Moore Creations without permission.
