IMPORTANT Shipping Requirements: Please Read and Follow.
1. Clean and sterilize all instruments prior to shipping. Remove all color bands and/or tape from instruments (OSHA regulations require all instruments be shipped in sterilized pouches.) All Color Band and/or Tape Removal will be charged $3.50 per instrument. If any instruments are sent in not cleaned and containing any debris, hair, bioburden or excessive staining on them there will be a $6 fee charged per instrument for extensive cleaning prior to servicing. Color bands are not returned. These fees are applicable to ALL Securos and non-Securos branded instruments.
2. Wrap any sharp instruments with extra padding to prevent damage during shipping.
3. Individually wrap all clipper blades to ensure they don't break during shipping. Do not autoclave the clipper blades for shipping.
4. You will be sent the RA# within 24 - 48 hours of submitting this form.
5. You MUST include a copy of this Instrument Work Order form with the packed instruments. Please include all pages.
6. Ship to: SECUROS, Attn: Repairs/RA#_____, 443 Main Street,Fiskdale, MA 01518 or request a Free UPS Ground Shipping Label on this form.
7. Charges will be applied for all outbound shipping. Please note if Express Shipping services are needed, otherwise all shipments go out with Ground Service.

Instruments are consistently evaluated for proper function throughout the repair process. Instrument flaws and defects may be found at any point in the repair process and the instrument rejected from any further repair process.
