I, the undersigned, do hereby consent and authorize Operation Never Forgotten ("ONF") and its affiliated media companies to use and reproduce my name; photographs; and any personal, biographical or medical information provided by me to ONF ("personal materials") and circulate the same for publication on the ONF website or on ONF circulars, billboards or other ONF media or documentation. The ONF website containing my name, photographs, and personal materials may be used and published, in whole or in part, in any and all media.

ONF is a charitable 501(c)(3) organization and does not engage in partisan political activities. I acknowledge that my name, photographs and personal materials may be used by ONF in whole or in part or not at all and the personal information may be paraphrased, abbreviated, edited or revised to suit the requirements or needs of ONF. If any items of personal materials provided by me to ONF have been previously prepared or published by any other third party, I will obtain permission from that third party for ONF to use said items.

I understand and agree that I will not receive any monetary compensation for the use by ONF of my name, photographs or personal materials. My consent for the use and publication by ONF of my name, photographs and personal materials will continue in full force and effect until revoked by me in writing which revocation will become effective on the date it is delivered to ONF.

I further release and indemnify ONF and its employees, officers, agents and affiliated companies from any and all claims of damages for libel, slander, invasion of rights of privacy or any other claims based on, arising from, or connected with the use of my name, photographs and personal materials.

I certify that I am over the age of 18 years and have the right to make this consent and release. This release shall be binding upon me and my heirs, legal representatives and assigns.
